Abstract | 成人開心術後症例を対象に静脈内投与および長期連用が可能な末梢血管拡張剤であるphentolamineと静注用nitroglycerinの血行動態に対する効果の比較検討を行った. ABP, LAP, RAP, およびPAPは, 両者の投与によりいずれも有意に低下した. しかし, phentolamine投与では, SVRIは有意に低下, SIおよびCIは有意に増加し, 動脈抵抗血管拡張による後負荷の著明な軽減が認められた. 一方, nitroglycerin投与では, 後負荷軽減ではほとんどなくSVRIは不変で, SIおよびCIはむしろ減少した. 心室仕事量は, phentolamineでは増加し, nitroglycerinでは静脈容量血管系の拡張による前負荷軽減作用を反映してLVSWIおよびRVSWIは減少した. またnitroglycerin投与による血液酸素化の悪化や, 投与中止によりすべての圧データの上昇やSVRIの上昇等のrebound現象が認められた. |

Abstract | A comparative study on the hemodynamic effects of vasodilator drugs of phentolamine and nitroglycerin which are capable of intravenous as well as long term administration were carried out on those patients who received open heart surgery. ABP, LAP RAP and PAP values showed significant decrease in both groups. However in phentolamine administration, SVRI was significantly decreaed, SI and CI were significantly increased due to decrease of after-load accerated by the dilatataion of arteriolar-restance vessels were observed. Contrally in nitroglycerin administration, there was no apparent decrease of after-load, SVRI did not change, SI and CI were rather decreased. The ventricular working volume was increased in phentolamine group, decreased in LVSWI and RVSWI in nitroglycerin group which were reflected by the reduction of pre-load due to the dilaton of venous capillary beds. Also nitroglycerin group, the reduction of blood oxygenation and the rebound phenomenon such as increase of all the pressure values and SVRI by the cessation of administration were observed. |
