| Abstract | 3例の褐色細胞腫患者を含む4例の高血圧患者に対しフェノキシベンザミン（POB）に代わる薬剤として塩酸プラゾシンを投与した. 術前の血圧安定化の目的で投与した2例の褐色細胞腫患者においては手術前日には血圧は正常レベルで安定し満足すべき結果がえられた. 術中術後経過もほぼ問題なかった. 一方, 極端に高いカテコーラミンレベルを示した悪性褐色細胞腫患者とカテコーラミンレベルが正常であった腎性高血圧患者各1例においてはプラゾシンの降圧作用は不十分であった. プラゾシンは褐色細胞腫の術前管理上POBに代わって用いる薬剤として有望である. ただしPOBに比較して, 1）投与量が確立されていない, 2）効果持続時間が短い, 3）効力がmildでありtachyphylaxisの傾向が強い, ことが問題と考えられる. また初回投与後の急激な血圧下降に注意を要する. はじめに 近年褐色細胞腫の麻酔管理は比較的安全に行えるようになった. これは術中管理技術の向上よりはむしろ術前管理の進歩によるところが大きいといわれる1,2）. |

| Abstract | Two cases with benign, resectable pheochromocytoma and a case with renovascular hypertension were given prazosin orally for the preoperative management. The cases with pheochromocytoma reacted favourably to prazosin and the hemodynamic control during nitrous oxide-morphine anesthesia was easily performed. However, in a patient with renovascular hypertension the effect of prazosin was insufficient. Another case with malignant pheochromocytoma was also treated with prazosin but the response was insufficient. We conclude that prazosin is（could be）a good substitute for phenoxybenzamine in the preoperative treatment of pheochromocytoma. |
