Abstract | はじめに Coronary arterial spasmは, 従来異型狭心症の病因として注目されていたが, 近年, 心内膜下虚血, 重篤な不整脈, 心筋硬塞あるいはsudden deathの一因となっていることが, 明らかになってきている1〜9). われわれは, 開心術後症例で, coronary arterial spasmにより重篤な左室機能の低下をきたしたと思われる2症例を経験したので, その原因および治療等について, 考察を加え報告する. 症例1. M.U. 61歳 男性 身長168cm 体重61kg 体表面積1.68m2 病名:不安定狭心症. 手術:大動脈-冠動脈(左前下行枝)バイパス術(A-C bypass). 既応歴:高血圧, 糖尿病. 家族歴:特記すべきことなし. 現病歴:昭和55年12月5日夜間睡眠中に初めて胸痛発作が出現し, 以後発作は労作時, 安静時にかかわらず頻回に起こるようになった. |

Abstract | Transient myocardial ischemia, probably due to coronary arterial spasm, occurred in two patients after open heart surgery. Marked ST elevation in V1 to V4 induced by inhalation of 100 per cent oxygen was associated with a severe depletion of left ventricular performance in. a 64 year-old man who had received A-C bypass. In a 47 year-old woman, who had received open mitral commissurotomy, a volus injection of CaCl2 was resulted in ST elevation in V2 to V5. During the ST elevation, her cardiac performance was markedly deteriorated. Continuous intravenous administration of nitroglycerin improved ST elevation, as well as cardiac performance, in both patients. Thereafter, oral administration of nitrol and nifedipine, Ca++ antagonist, helped to suppress coronary arterial spasm. |
