Abstract | 虚血性心疾患を除く(主として弁膜疾患)開心術前後の大動脈内バルーン・パンピング(IABP)施行症例26例を対象とし, その臨床成績, 血行動態等につき検討を加えた. IABP離脱率は64.3%(26例28回中18回)で, 26例中生存例11例(42.3%), 死亡例15例(57.7%)であった. IABPは弁膜疾患に対しても有効であり, 左心系パラメータの改善に伴い右心系のパラメータも改善し, さらに難治性不整脈にも有効であった. また, IABP離脱後の死亡例が多く, 複雑な臨床像を呈する重症弁膜疾患症例に対する施行中および離脱後の管理の重要性が示唆された. |

Abstract | A clinical and hemodynamic studies on pre-and postoperative application of intra-aortic ballon pumping(IABP) wre undertaken on 26 patients mainly consisted with valvular heart disease without ischemic heart disease. Overall successful rate of weaning from IABP was 64.3%(18 out of 28 occasions) and mortality was accounted as 57.7%(15 out of 26 patients). Our experience showed that IABP was effective for the treatment of valvular heart disease after surgery well as for intractable arrythmia, however, many patients died after weaning from IABP. These facts would suggest that careful management during and after weaning from IABP, are most important for the treatment of severe valvular heart disease who are manifesting with complicating clinical pictures after surgery. |
