| Abstract | 「要旨」開心術後の不整脈の発生状況と原因の検討, およびその治療方法としてのペーシング療法の検討を行った. 最近の360例の開心術後症例のうち169例(47%)に合計242エピソードの不整脈発生をみた. 先天性心疾患群では, 各種の上室性不整脈および房室ブロックが特徴的で, 後天性心疾患群では, 心房細動を基礎として徐脈や心室性期外収縮の発生が特徴的であった. 不整脈発生は, ASD群では, 高齢, 高シャントパッチ使用群に多く, 弁置換群では, 高齢, 高心胸比, 高NYHA, 低カリウム血漿群に多かった. 治療法としては, 各種の新しいペーシング方法を採用し, 有効性を確認した. 「はじめに」種々の先天性ならびに後天性心疾患の開心術治療に際して, 刺激の発生および伝導に対する障害は避けうべからざる合併症のひとつである. その多くは一過性に治癒するが, 術中術後の不安定状態に発生するものは, 時には重篤な状態を引き起こしかねず, その発生原因と治療に関しては強い関心がよせられている20). |

| Abstract | 360 cases of open heart surgery recently undertaken were analyzed to determine the incidence, type, etiology and pacing therapy of rhythm disorders. Arrhythmias appeared in 169 cases (47%) of the 360 cases studied. There were higher incidence of supraventricular arrhythmias in patients with congenital heart disease, and ventricular premature beat with acquired heart disease. Some factors were searched out for the etiology of arrhythmias in patients with ASD and valvereplacement. Almost all kinds of arrhythmias were treated by cardiac pacing which include newly applied techniques. |
