| Abstract | 「要旨」帝王切開時に見られた高血圧に対して静注用ニトログリセリン(TNG)で治療した11例の経過を報告し, 妊娠中毒症における循環制御について考察した. TNGの投与は1回量0.25mgの間歇的静注にて行った. 特に7例の重症妊娠中毒症の症例では, 気管内挿管の直前にもTNGを投与して血圧の異常上昇を予防した. 児娩出後は子宮収縮剤としてオキシトシンを持続静注した. 以上の管理方法で母体の血圧はほぼ十分に調節できた. 新生児においてもTNGによると考えられる副作用は認められなかった. TNGとオキシトシンは妊娠中毒症における循環制御の面で有用である. ただしTNGの母体と児に対する作用のより詳細な検討は今後の課題である. 「はじめに」重症妊娠中毒症で高血圧(以下「中毒症性高血圧」)を呈する妊婦は胎盤早期剥離や胎児仮死などにより緊急の帝王切開(以下「帝切」)を要することが高頻度である1). そのさいの麻酔法としては全身麻酔の選択を余儀なくされることが多いが, 気管内挿管直後と児娩出直後の2つの時点で中毒症性高血圧は更に増悪する危険性が高い. |

| Abstract | Eleven parturients showed remarkable hypertension during general anesthesia for cesarean section, and were treated with bolus injection of nitroglycerin (TNG.) In seven severe preeclamptic patients, 0.25mg of TNG was given just before tracheal intubation to decrease the pressor response. After the delivery, drip infusion of oxytocin (20mIU/min.) was give in place of ergometrine derivatives. Hypertension was well controlled throughout the anesthetic procedure. No serious side effect of TNG was found in the neonates. It is concluded that TNG and oxytocin are good agents for circulation control in preeclampsia. However, the effect of TNG on maternal intracranial pressure and fetal or neonatal circlation should be investigated further. |
