Abstract | 「要旨」脳血管に対する頸部交感神経系の関与についてはまだ一定の結論が得られていないのが現状である. 頸部交感神経遮断の脳血管に与える影響を検討するために虚血性脳血管障害患者の浅側頭動脈-中大脳動脈皮質枝吻合術術中に頸部硬膜外ブロックを行ない, そのブロック前後の中大脳動脈皮質枝の血管径, 血流量の変化を調べた. 1%メピカイン10mlの頸部硬膜外ブロックにより中大脳動脈皮質枝の血管径は23%, 血流量は51%増加した. 虚血性脳病変や脳動脈瘤破裂後の血管攣縮に対して上頸部交感神経節切除の有効性が報告されており, 頸部硬膜外ブロックにても上頸部交感神経節切除と同様の効果が期待出来ると思われる. |

Abstract | The widely accepted conclusion regarding the function of cervical sympathetic system onto cerebral vessels has not yet been obtained. In order to investigate the effect of cervical sympathetic block on to cerebral vessels, the diameter of the vessel and amount of blood flow at cortical branch of middle cerebral artery were measured before and after cervical epidural block during STA-MCA anastomosis. Both diamater of the vessel and blood flow increased 23% and 51% respectively by cervical epidural block with 10 ml of 1% mepivacaine. There have been some article reporting the effectiveness of superior cervical sympathectomy to the ischemic cerebral disorders or to the vasospasm following the rupture of cerebral aneurysm. Cervical epidural block was more simple to perform but also seemed to bring the almost same effect as superior cervical sympathectomy. |
