Abstract | 「要旨」スワン・ガンツカテーテルの普及は術中・術後の患者管理に大きな貢献をしてきている. 一方では, カテーテル使用に伴う種々の合併症が報告されている. 今回, われわれは, スワン・ガンツカテーテルが蛛膜下腔に挿入された貴重な症例を経験した. バルーンを膨らませた時の右足の動き, 胸部及び頸部X-P, コーディス式インジューサーから得られた液の検査所見から上記合併症が確定された. 幸い, この症例では何ら後遺症を残すことなく退院できた. 本症例を通して, 正しい技術の修得及びスワン・ガンツカテーテル使用の適応決定の重要性が痛感された. 近年, 手術及び麻酔中の心機能を評価する目的でスワン・ガンツカテーテルを使用しての観血的モニタリングを用いることが多くなってきた. それに伴いカテーテル自身の問題, 及び, カテーテル挿入の手技に伴う合併症が数多く報告されている1)〜4). 今回, 我々は, スワン・ガンツカテーテルを誤って蛛膜下腔に挿入した症例を経験したので報告する. |

Title | An inadvertent insertion of a Swan-Ganz catheter into the intrathecal space : a rare complication associated with catheterization of the internal jugular vein |

Abstract | A rare complication case of percutaneous catheterization of internal jugular vein was experienced in a 45 year-old woman underwent a total gastrectomy for advanced gastric cancer. Pulmonary artery pressure monitoring was indicated for proper fluid management during anesthesia. It was planed to insert a Swan-Ganz catheter through the right internal jurlar vein. The catheter was advanced about 20 cm from the skin. When the balloon was inflated with 0.8 ml air, suddenly the patient's right leg was twiched. Then an inadvertent insertion of the catheter into intrathecal space was suspected. A chest X-ray was immediately performed. The X-ray showed the Swan-Ganz catheter in the spinal cannal. The catheter was extracted immediately. After the operation the patient was examined by neurologists. Although motor neuron function was intact sensory neurons were slightly injured. The nerve injury improved progressively and no neurological deficit was observed at the third postoperative day. This case directs us to two meaningful problems. |
