| Abstract | 「要旨」本研究の動機は, 大動脈瘤手術で下行大動脈を遮断したところ心停止に陥った症例の麻酔管理を経験したことに始まる. 心臓後負荷の急激な増加が心停止の原因と考え, 雑種成犬で実験を行った. 結果は, 大動脈遮断時および解除時の遮断中枢側の血圧変動が, 下大静脈血流を制御することにより防止し得る, ということであった. 理論的には, 大動脈遮断および解除は血液の下半身から上半身への移動をもたらすことから, 後負荷の変動と本質的には同じであること, 後負荷の変動による平均循環圧の動きは前負荷変動により制御しうる, と考察された. 最後に, Starling curveの数式化を菅らの理論により行った. 後負荷と心収縮力を因子に持つ本数式により, 心拍出量と後負荷は生理的範囲内では相互関係が明らかになった, と結論できた. 「1. 研究の動機」胸部大動脈瘤手術は, 今だ手術成績は芳しくなく, 術中死も稀でない, 当施設でも. 近年ようやく, 成功率が50%を超えるようになったが, 弓部に病変があるものでは成績は甚だ劣悪である1). |

| Abstract | The motive of study about cardiac output and afterload relationship was obtained from the a patient, who had cardiac arrest by aortic cross-clamping in the operation of the aortic aneurysma. Through the investigation of mongrel dogs' aortic clamping and declamping, we found that the change of systemic pressure could be well controlled by the regulation of inflow to right atrium. Theoretically, aortic clamping and declamping are identical with the change of afterload or preload, that is, the blood centralization and decentralization. The different point between afterload and preload is as follows. On venous return curve by Guyton, the change of the former causes less movement of Pms and the slope than the latter. Finally, we presented the formula representing Starling's curve which included factors of myocardial contractility and afterload. We conclude that the relationship between cardiac output and afterload can be determined by our presented formula. |
