Abstract「要旨」脳腫瘍の異常血管網や頭蓋内静脈環流遅延が存在する場合の頭蓋内圧亢進病態において圧波の出現に関する検討を行なった. 症例は43才男右前頭葉glioblastoma, 49才男多発性転移性脳腫瘍及び42才と51才女 bengin intracranial hypertension(BIH)の計4例でCTでは全例広汎な脳浮腫の所見を呈した. 転移性腫瘍例は頭蓋内圧モニター中死亡し他は治療後軽快するも再発, 心不全, 不明原因などで死亡した. 頭蓋内圧は67-314時間モニターし, 測定範囲は3.6-100mmHgで, A波の出現は33-59回/24時間であった. 治療にもかかわらずA波は睡眠, 覚醒両期に呼吸異常を伴わなくても出現し, 転移性腫瘍例では圧波出現の基礎圧レベルが上昇し, A波が多発し突然死亡した. Glioblastomaの標本で血管の微細構造を調べると管径不同で太く蛇行する血管から細い吻合枝が多数生じ血管網を形成し, BIH例では脳血管撮影にて上矢状洞造影不良や静脈造影遅延所見が認められた.

AbstractThis clinical study was designed to clarify the mechanism of the appearance of large pressure waves in these cases. Four cases with frequent appearances of pressure waves were studied. They were two adult cases of brain tumor (right frontal glioblastoma: one case, multiple metastatic brain tumors: one case) and other two cases of benign intracranial hypertension (BIH). The patient with multiple metastic tumors died from respiratory arrest complicated with gastrointestinal bleeding. The other three cases were improved transiently after surgical treatment. However, they died from reccurrence of tumors or from cardiac failures. Changes of ICP ranged between 3.6 to 100 mmHg. The number of appearances of A waves distributed from 33 to 59 times per 24 hours. Pressure waves appeared frequently during waking as well as sleep states. Abnormal respiratory movements were not always associated with A waves. However, at the end of A waves hyperventilated states were occasionally observed. Vascular conformations were evaluated in the case of glioblastoma by the method of Hasegawa and Ravens (1968).
