Abstract | 中大脳動脈閉塞や狭窄の20例(55.3±7.4才)の慢性期の脳血管撮影(CAG), CTや局所脳血流(rCBF)所見から脳循環動態を検討した. rCBFはDI(diffuse ischemic pattem 全般減少)型:Fg55.5±5.2(SE)ml/100g/min 6例;FI(focal ischemic pattern 部分減少)型71.6±5.9, 10例;NP(normal pattern 正常)型81.8±7.4, 4例に分類された(各群間P<0.001). DI型では基底核や脳室周辺に低吸収域が多発し血管狭窄も高度でcollateral flow(CF)も発達不良であった. FI型では血管狭窄も種々で脳室周辺や脳表にLDA(CT上のlow density area)が多くNP型と同様CFは発達良好であった. 臨床症状は必ずしもrCBF低下の程度と一致しなかった. rCBF減少の要因としてdeactivationによる代謝低下, CF発達の差異及び局所CF阻害因子などが考えられた. |

Abstract | Twenty patients of mean 55.3 years-old with chronic occlusion or stenosis of middle cerebral artery were included in this study. Their cerebral hemodynamics were estimated by measurements of regional cerebral blood flow(r-CBF;Fg) and findings of cerebral angiography as well as CT. Their neurological states were complete stroke(11 cases), RIND(7) and TIA(2). Patients were classified into three categories from significant differences in r-CBF values. Diffuse Ischemic pattern(DI:6 cases), Focal Ischemic pattern(FI:10 cases) and Normal pattern(NP:4 caese). R-CBF values were most lowest in DI, while those in FI were lower than NP. CT-lesions seated in basal ganglias were frequently found in DI group followed by FI group. But none of NP group showed such a lesion. Severe stenosis or poor development of collateral flow were most prominent findings in DI group. A low density area in watershed zone was distributed equally through 3 groups. However, degrees of symptoms were not so well correlated with decreasing of rCBF. |
