Abstract | 成人開心術後数時間の血行動態が安定した時期に, 硝酸イソソルビド(ISDN)を2.5μg/kg/min持続静脈内投与を行い, 投与前, 投与開始後2時間, 投与中止後2時間の血行動態の変化からISDN持続静注の効果の検討を行った. ISDN投与中の動脈圧, 左房圧, 肺動脈圧, 右房圧, SVRI, PVRI, LVSWI, およびRVSWIは, ISDNを投与しない時期に比し, いずれも有意に低値を示した. 心拍出量は有意の変化を示さなかった. ISDN投与中の動脈血酸素分圧はISDNを投与しない時期に比し, 有意の低値を示したが動脈血炭酸ガス分圧は有意の変化を示さなかった. これらの結果より, 末梢血管拡張療法として, ISDN持続静脈内投与は両心の前負荷および後負荷の軽減作用が認められ, 有用であると考えられた. |

Abstract | Adult patients received isosorbide dinitrate(ISDN) at a dose of 25μg/kg/min by intravenous administration when their hemodynamic condition was stabilized several hours after open-heart surgery, and the changes in their hemodynamic parameters were determined 2 hours after the start of intravenous administration and 2 hours after drug discontinuation in order to evaluate drug effects in open-heart surgery patients after intravenous administration. Arterial pressure, left atrial pressure, pulmonary arterial pressure, right atrial pressure, SVRI, PVRI, LVSWI, and RVSWI were significantly decreased after dose administration, compared to pre-drug values, but there was no significant change in cardiac output. Arterial partial pressure of O2 during dose administration was significantly lower than before dose administration, while arterial partial pressure of CO2 showed no significant change after dose administration. Considering that ISDN reduces pre- and after- load of both ventricles after intravenous administration, our results suggest that this drug provides a useful therapeutic for peripheral vasodilation. |
