| Abstract | 試作したソフトを用いて, 指尖容積脈波の伝達時間, 波形面積を求めることによる解析を, 他の循環系パラメーターと比較検討し, 試みた. 対象は開心術, 並びにシャント術麻酔症例で, 末梢循環の変化として導入前, 刺激侵襲の加わった時点, 安定時, 人工心肺離脱後の4点で測定を行なった. 刺激侵襲の加わった時点は導入前に比し, 波形面積が-42±7.0％と有意に抑制され, 末梢循環障害を呈している. 安定時には15±40％とばらつきが大であるが, 様々な要因の影響をこの時点で受ける為と思われる. 又, これらの結果と術前心機能との間に相関は認められず, フェンタニール, モルヒネ間, 並びにチアノーゼ疾患と非チアノーゼ疾患との間に有意な差は認められなかった. 以上より速時性の高い末梢循環の評価が今回の方法で得られ, 薬剤投与時の血行動態に及ぼす効果判定に有益であると判断された. |

| Abstract | We had tried an analysis of pressure wave and plethysmogram wave by soft programs manufactured for trial. Objects of this study are patients of cardiovascular disease. The tephron needle was inserted under consciousness in the radial or ulnar artery. At the same time, plethysmogram pickup attached on the finger of the hand. Pulse wave, pressure wave and ECG−II lead was recorded in data recorder. Recorded parameters were analysed by producted programs for trial. Results of analysis are expressed as percent changes. It was investigated about a relation of preoperative cardiac parameters and analysed data. It was also studied on the fentanyl and morphine anesthesia. The characteristic changes were not observed on two anesthesics. But effects of anesthetics for vascular bed is easily visible. And this method is effective for estimating the periferal circulation. |
