Abstract | 心肺機能に異常を有しない手術患者6名を対象とし, 心エコー図法およびスワンガンツカテーテル法にて, ハロセン麻酔下における循環動態の変化を検討した. 心エコー図法より計測された左心室の駆出状態を表す指標(Ejection FractionおよびFractional Shortning)は, 1MACに比べ2MACにおいて有意に減少していた. また同時に測定した左室容量において, 左室収縮期容量が有意の増加を示していた. 以上の結果より, ハロセンは濃度依存性に心収縮性を減少するものと思われた. さらにスワンガンツカテーテルより得られた肺動脈契入圧も, 濃度依存性に有意に上昇しておりこれを示唆すると思われる. |

Abstract | Echocardiographic studies were made of 6 hearthy patients scheduled for surgical procedure to evaluate the effect of halothane on left ventricular function. Using M−mode echocardiography we found that 1 and 2 MAC halothane induced dose−related decrease in left ventricular function. Two ejection−phase performance variables, Ejection Fraction and Fractional Shortning decreased to 0.70+0.08 and 33.7+6.2% (mean+SD) at 2 MAC halothane from control values of 0.74+0.08 and 36.6+6.9% at 1 MAC halothane. Dose−dependent depressant effects of halothane on the heart were accompanied by increased left ventricular end−diastolic pressure. Through the Frank−Starling mechanism, the increase in left ventricular end−diastoric pressure should have increased left ventricullar function. We should preserve left ventricular end−diastoric pressure at higher levels for the maintanance of cardiac output. |
