Abstract | 心臓血管術後患者において心拍数の安定化は有効な心拍出量を得ることからも非常に重要な因子のひとつである. しかし, 各種薬剤に抵抗する頻脈発作に遭遇しその治療に難渋する場合がある. 今回我々は心臓血管術後患者12例に発生し強心配糖体やディソピラマイドに抵抗を示した難治性頻脈に対してslow channel blockerのひとつである塩酸ジルチアゼムを心筋収縮力抑制作用が有意に発現しない様な低濃度(0.8〜2.0μg/kg・min)にて持続静脈内投与を行い, 投与前値と投与開始約1時間後の心拍数を比較検討することにより良好な結果を得たので若干の考察を加えてここに報告する. |

Abstract | Diltiazem hydrochloride, a slow channel blocker, is known to have various effects on cardiovascular system such as reduction of systemic vascular resistance, diltiazem of coronary artery, suppression of myocardial contractility and decrease of heart rate. This study was undertaken to evaluate the efficacy of intravenous administration of diltiazem hydrochloride (CRD 401) on refractory tachyarrythmias following cardiovascularsurgery (CVS). CRD 401 was administrated continiously by intravenous infusion at the rate of 0.8 to 2.0 μg/kg・min on 12 patients who were suffering from an intractable tachyarrhythmia after CVS. The type of tachyarrhythmias were atrial fibrillation or fiutter in patients (group A) and supraventricular tachyarrythmia in 5 patients (group S). These tachyarrythmia were not controlled by other antiarrhythmic drugs. A marked reduction of heart rate was obtained in all patients within an hour of the administration of CRD 401 without showing any compromise on cardiac performance. |
