Abstract | 3才から9才までの小児39例を対象に麻酔導入時および気管内挿管時の循環動態の変化を超音波ドプラー法(UltraCOM(R))により連続的に測定し, 以下の結果を得た. ハロセン導入群(19例), サイアミラール導入群(10例)では, サクシニルコリンおよび気管内挿管により, 主に脈拍数の増加に依存した20〜30%の心拍出量の増加がみられた. 一回心拍出量の変化は小さかった. この心拍出量の増加は, フェンタニールを少量投与した群(10例)では認められなかった. 小児の麻酔導入・挿管時においても循環動態の変動は大きく, フェンタニールの投与はこの変動を抑えた. |

Abstract | Measurements of hemodynamic changes (Heart Rate(HR), Cardiac Index(CI), Stroke Index(SI)) were performed in 39 surgical children (averaged aged of 5.6 years old), by a non−invasive cardiac output measurement utilizing ultrasonic doppler method(UltraCOM(R)). They were divided into 3 groups, halothane slow induction (0.5−2.0%) group, thiamylal rapid induction (3−4mg/kg) group and fentanyl pretreatment (2−3μg/kg) group. In each group succinylcholine, chrolide (SCC) 1mg/kg were given to facilitate intubation. The following results were obtained. 1)In halothane (19 cases) and thiamylal (10 cases) group, HR and CI increased (20−30%) significantly after both SCC administration and endotrachal intubation. SI had a tendency to increase (12%) in halothane group after intubation and to decrease (9%) after SCC administration in thiamylal group. 2)In fentanyl group (10cases), HR and CI decreased after induction. |
