Abstract | 一般手術に対して各種麻酔法で術中管理を行っていた間に発症した高血圧に対して, 塩酸ジルチアゼムを投与し, その降圧効果と心血行動態に及ぼす影響を検討した. 患者をジルチアゼム10mg一回静注群と持続点滴群に分け, 投与前後の血圧, 心拍数, を測定した. また一回静注群のうち6例にSwan−Ganzカテーテルを挿入し, より詳細な血行動態を検討した. 両塁塞ジルチアゼム投与後速やかに血圧が下降したが, 持続点滴群の方が, 降圧効果に持続性があり, 心拍数も減少させず, より安全で好ましいとの印象を受けた. またジルチアゼムは, 一回静注により血圧は下降するものの心係数や一回拍出量係数は上昇し, RPP, 末梢血管抵抗は下降した. これらのことはジルチアゼムは, 虚血性心疾患を持った高血圧患者に対しても安心して使用できることを示唆するものと思われる. |

Abstract | Diltiazem HCl was administered to patients exhibiting hypertension during operative management with various anesthetic methods in general operation, and the hypotensive and hemodynamic effects of the drug were investigated. Patients were divided into a single intravenous group of diltiazem (10mg) and a sustained infusion group, and blood pressure and heart rate before and after the administration of the drug were measured. A Swan−Ganz catheter was inserted and more detailed hemodynamics were examined in six patients in the single intravenous group. In both groups, an immediate lowering of blood pressure was seen after the administration of diltiazem. However, safer and better impressions were obtaind in the sustained infusion group than in the single intravenous group, the former showing a longer duration in hypotensive effect and no decrease in heart rate. Although a single intravenous administration of diltiazem decreased blood pressure it increased cardiac index and stroke volume index, and decreased RPP and peripheral resistance. The results suggest that diltiazem can be used safely in hypertensive patients with ischemic heart diseases. |
