Abstract | 房室伝導に対するフェンタニールの作用とベラパミルとの相互作用を検討する目的で, イヌ29頭を使用し, フェンタニール20μg/kg, ベラパミル100μg/kg, アトロピン0.1mg/kg, ないし0.4mg/kgの投与方法により4群に分け, さらに迷走神経刺激のモデルとしてネオスチグミンを単独投与した1群を加えて, 合計5群に分けた. 観察したパラメータは自発洞周期(SCL), 房室結節内興奮伝導時間(AH), 房室結節有効不応期(FRP)などである. フェンタニールによりSCL, AH, FRPは有意に延長し, アトロピン0.4mg/kg, を同時に投与することにより完全に拮抗された. また, ネオスチグミン0.1mg/kgによりフェンタニールと同様の効果を得ることができた. ベラパミルはSCLに対しては一定の効果は持たないが, AH, FRPに対しては延長効果があり, フェンタニールと併用した場合には相加的効果がみられた. |

Abstract | Verapamil is used for treatment of both hypertension and dysrhythmias during anesthesia. Although it is well documented that fentanyl stimulates vagal tone, there are only few reports on interaction of fentanyl with verapamil which has inhibitory effects on AV conduction. Therefore, the present study was designed to evaluate the combined effects of fentanyl and verapamil on sinus and AV nodal function. Twentynine mongrel dogs were divided into 5 groups. The following parameters were measured;the sinus cycle length(SCL), atrium−His(AH)interval, functional refractory period (FRP) of AV node, etc. All parameters except SCL were measured in the basic cycle which was obtained by atrial pacing on 330 msec intervals. After control measurements, all measurements were repeated 5 minutes after intravenous administration of following drugs;(I) fentanyl->verapamil (II) verapamil alone (III) neostigmine alone (IV) atropine->fentanyl->verapamil->atropine (V) atropine+fentanyl->verapamil. Atropine 0.4mg/kg completely inhibited the prolongations of SCL by fentanyl. |
