Abstract | 体外循環を用いた開心術においてプロスタグランディンE1(PGE1)の血清補体活性に与える影響を調べた. 18例の弁置換患者を2群にわけ, 9例では体外循環時に初回量100μgのPGE1を投与し, 以後2.5μg/minで滴下した(投与群). 他の9例を非投与群とし, 各々体外循環前後のC3a, C5aの血清レベルを測定し比較した. 体外循環終了直前におけるC3aの上昇は, 体外循環前と比較すると, 投与群では2.49倍(552±197から1373±408ng/ml)の上昇を示し, 非投与群の4.61倍(481±210から2218±754ng/ml)に対し有意に低値を示した(p<0.01). 一方C5aは統計学的に有意差は認められなかったが投与群で1.79倍上昇したのに対し, 非投与群で4.86倍であり投与群において低い上昇であった. 以上の結果から体外循環におけるPGE1投与は補体活性に抑制的に働くものと思われた. |

Abstract | Effects of prostaglandin E1 (PGEI) on complement activation were evaluated in patients who underwent valvular replacement surgery under cardiopulmonary bypass. Eighteen patients were divided into two groups equally. Nine patients were administered PGE1 during CPB period. The control group were managed similary but without the administration of PGE1. In all the cases, total CPB was applied. In prime perfustes 4mg of sodium betamethasonephosphate was added. In PGE1 group each patient had approximately 400μg PGE1 during CPB. Hundred micrograms PGE1 was initially administered in the prime perfusate and it was titrated at the rate of 2.5μg/min. The change of C3a from preoperative value to the end of bypass was 2.49−fold(552±197 to 1373±408ng/ml, mean±sd) in the PGE1 group in contrast to the 4.61−fold increase(from 481 to 2218ng/ml) in the control group. And there was statistically significant difference between the values at the end of bypass between the two groups (p<0.01 by Wilcoxon two sample test). |
