Abstract1984年8月から1986年11月までに, 国立札幌病院で施行された27例の乳児期緊急心臓血管手術について, 術前術後の合併症と死亡率について検討した. 症例は生後3か月以内のものが81%を占め, 平均体重は3825gであった. 原疾患は多岐にわたったが, 開心術は6例で, 他はすべてシャント術, 肺動脈絞扼術などの非開心術であった. 術前合併症では黄疸が多く, ショック状態や多臓器不全に陥っている症例もみられた. 術後合併症では3例にカテーテルに起因する敗血症を, 2例に肝不全をみた. ほかに, 緊張性気胸, 心嚢液貯留, 横隔神経麻痺などがみられたが, 肝不全の2例を失ったのみで, そのほかの合併症は改善しえた, 全体の死亡率は30%(8例)で, うち6例が術中または術後24時間以内の死亡であった.

AbstractA retrospective analysis of hospital courses of 27 infants underwent emergency cardiovascular procedures at the National Sapporo Hospital duing current 28 months was made in order to evaluate perioperative complications and mortality. Patients aged between two days and four months. The average weight was 3825g. Closed procedures such as shunt procedures were performed in 21 cases and open heart procedures were done in 6 cases. Preoperative complications were seen in 9 out of 27 cases (33%). They included icterus (4 cases), acute renal failure (2), cardiogenic shock (2), atelectasis, convulsion, and pericardial effusion. Postoperative complications were observed in 14 cases (52%). Major complications were sepsis (3), hepatic failure (2), tension pneumothorax, and pericardial effusion, while minor ones included wound infection (2), atelcetasis (2), phrenic nerve palsy (2), and liver dysfuncton.
