| Abstract | DBcAMPを10日間以上の長期間, 低濃度持続投与（5.6±2.2μg／kg／min）した19例中11例に高カルシウム血症（12mg／dl以上）を認めた. DBcAMP投与中止により, 尿中無機リンの排泄は有意に低下し（17.4±6.8から9.8±8.6mg／kg／day）, カルシウムの排泄は増加する傾向があった（4.5±3.3から5.3±3.5mg／kg／day）. また血中副甲状腺ホルモン濃度は有意な変化を示さなかった（0.42±0.18から0.44±0.28ng／ml）, 低濃度持続投与であっても, 長期間にわたってDBcAMPを投与する場合は高カルシウム血症を来す可能性があることを考慮し, 血清カルシウム濃度が上昇傾向にあるときは速やかに投与量を減量するか, 投与を中止する必要があると思われる. |

| Abstract | Continuous infusion of DBcAMP has been recommended for the treatment of low cardiac output state after cardiac surgery. But hypercalcemia （more than 12mg／dl) had occcured in eleven of 19 patients after long−term use （more than 10 days） with low concentration of DBcAMP（5.6±2.2μg／kg／min）. In eight of 11 patients, from whom we could get data in detail to analyze, the highest level of serum calcium was 14.0±1.5mg／dl. Serum calcium level had decreased and serum inorganic phosphorus level increased to the normal ranges immediately after cessesion of infusion of DBcAMP. Urinary excretion of calcium had increased （from 4.5±3.3 to 5.3±3.5mg／kg／day） and that of inorganic phosphorus had significantly decreased （from 17.4±6.8 to 9.8±8.6mg／kg／day） after stopping DBcAMP. But serum parathyroid hormone level did not change （from 0.42±0.18 to 0.44±0.28ng／ml）. We have to pay attension to serum calcium level when we use DBcAMP for a long time despite continuous infusion with low concentration. |
