Abstract | 実験的出血性乳酸アシドーシスを作製し, ピルビン酸デヒドロゲナーゼの活性を賦活して乳酸の利用を亢進させるdichloroacetate(DCA)の代謝, 心循環系に及ぼす効果をイヌを用いて検討した. 平均動脈圧を50mmHgに1時間保持する様に脱血すると, 血中乳酸, ピルビン酸, L/P比は有意に増加したが, DCA投与により時間経過と共に有意に低下した. 動脈血pH, HCO3-も対照値近くに回復した. 脱血により低下した平均動脈圧, 平均肺動脈圧, 肺動脈楔入圧及び増加した心拍数は還血により対照値近くまで回復したが, 心拍出量はDCA投与の場合には非投与群に比較してより改善された. DCAは出血に由来するタイプA乳酸アシドーシスの治療に有効と思われる. |

Abstract | Hemorrhagic shock has been known to decrease tissue perfusion and oxygenation and leads to severe lactic acidosis. We evaluated the effect of dichloroacetate(DCA), which enhance the pyruvate dehydrogenase enzyme activity, on lactic acidemia in experimentally produced hemorrhagic shock. The blood was shed from the femoral artery in order to obtain mean arterial blood pressure of 50mmHg for 1hr. In five dogs, DCA was administered i. v. together with retransfusion of shed blood, while in five dogs no DCA was given. Lactate and pyruvate levels were significantly decreased by DCA administration. Significantly decreased mean arterial and, mean pulmonary arterial pressures were restored by retransfusion of the shed blood and cardiac output was maintained during treatment with DCA. These data suggest that DCA administration improve hemodynamics and metabolism in lactic acidemia induced by hemorrhagic shock. |
