Abstract雑種成犬を用いて新しい陽性変力薬であるアムリノン, ミルリノン, Dibutyryl cyclic AMP(DBcAMP)の心機能, 冠循環動態に及ぼす影響について検討した. 体循環に関しては3剤は力価が異なるのみでいずれも陽性変力作用と後負荷減少作用を示した. 冠循環に関してはいずれも冠血流量の増加, 冠血管抵抗の減少, 心筋酸素消費量の増加を示したが, 心筋酸素摂取率はアムリノン, DBcAMPは減少したがミルリノンは不変であった. 従って冠血流量増加の機序としては, アムリノン, DBcAMPは冠血管拡張作用によって, 一方, ミルリノンは心筋酸素消費量増加に伴なう自動調節による可能性が大きいと考えられ, 冠血管に対する作用はミルリノンは他の2剤とは異なることが示唆された.

AbstractThe coronary hemodynamic effects of intravenous bolus injections of amrinone(1.5mg/kg), milrinone(50μg/kg) and DBcAMP (10mg/kg) were evaluated in 23 mongrel dogs under pentobarbital anesthesia. Coronary blood flow significantly increased by 33% in amrinone, 28% in milrinone and 25% in DBcAMP. Myocardial oxygen consumption significantly increased by 20% in amrinone, 26% in milrinone and 11% in DBcAMP. While coronary vascular resistance significantly decreased by 28% in amrinone, 23% in milrinone and 27% in DBcAMP. Myocardial oxygen extraction ratio which indicated myocardial oxygen demand−supply balances significantly decreased by 11% in amrinone and 10% in DBcAMP, but showed no change in milrinone. These results suggest that amrinone and DBcAMP had direct coronary vasodilating effects, while milrinone had no or small effects, because it showed no changes of myocardial oxygen balances. Therefore amrinone and DBcAMP might worsen myocardial ischemia in some patients with critical coronary stenosis owing to coronary steal.
