| Abstract | 手術中の出血量が1000ml以上と予測された成人定期手術症例15例において, 大量輸血時の血漿中幅ハプトグロビン濃度・血漿中総ヘモグロビン濃度および血漿中遊離ヘモグロビン濃度を輸血量ごとに測定したところ, 血漿中総ハプトグロビン濃度は輸血量1800mlの時点で有意に低下した（p＜0.01）. また, 血漿中遊離ヘモグロビンは血漿中総ハプトグロビン濃度が120mg／dl以下で出現した. 輸血量が1800mlを越えた7例において, ハプトグロビン製剤4000単位を投与したところ, 血漿中遊離ヘモグロビンは検出されなくなり, さらには血漿中総ハプトグロビン濃度が輸血前値あるいはそれ以上に回復した. 輸血量1800ml以上の症例では, ハプトグロビン製剤の補充療法は有用と思われた. |

| Abstract | It is reported that massive transfusion results in decreased plasma haptoglobin level due to increased free hemoglobin. Therefore, we measured the plasma haptoglobin, hemoglobin and free hemoglobin levels in 15 adult patients whose operative blood loss was predicted over 1000ml. The plasma haptoglobin level decreased significantly in patients who received 1800ml of transfusion. The plasma free hemoglobin level started to appear when the plasma haptoglobin level decreased to 120mg／dl. Intravenous administration of haptoglobin 4000 unit restored the plasma haptoglobin to the control level in patients who received over 1800ml of transfusion, and free hemoglobin became undetectable. Our results will suggest that administration of haptoglobin 4000 unit is reasonable in cases of massive transfusion over 1800ml during operation in order to prevent post−operative renal failure due to decreased haptoglobin and increased free hemoglobin in the circulating blood. |
