Abstract「要旨」血漿増量剤の循環動態に及ぼす影響をEar Densitography(EDG)を用いて検討を加えた. 予想出血量10ml/kgに相当する, 主に腹式子宮全摘出術を受ける28名の女性を対象とした. 血漿増量剤のサリンヘス(R)(S-HES), ヘスパンダー(R)(HES), サヴィオゾール(R)(D40R), およびその対象として生理食塩水(S)の4つの群に分けて比較を行った. 各群において投与前, 投与中, 投与終了後30分, 60分, 90分における, Systolic Time Intervals(STI)のうちPre-ejection Period(PEP), Left Ventricular Ejection Time(LVET), PEP/LVET(P/E), および各時点における血圧, 心拍数, 出血量, 尿量, 血清電解質(Na, K), ヘマトクリット値(Ht), 血清総蛋白濃度(TP)を求めた. D40LR, S-HES, HESはSと比較して, 投与中にいち早くLVETを延長させ, 一回拍出量を増加させていることが推測された. また, S-HESでは投与前値と比較して投与終了後60分, 90分でPEPの短縮とP/Eの減少がみられた. これは出血により循環血液量の減少を認める反面サリンヘス(R)を投与することにより前負荷が増加し, 心ポンプ機能の改善をもたらしたと考えられる, またEDGは麻酔中の循環モニターとして有用であった.

AbstractThe effects of plasma expander, Salinhes(R) (Hydroxyethyl starch in 0.9% salt solution, S-HES), Hespander(R) (Hydroxyethyl starch in balanced salt solution with 1% glucose, HES), and Saviosol(R) (3% Dextran 40 in Lactate Ringer solution, D40LR), on hemodynamics were examined using Ear Densitography (EDG) in 28 females who were scheduled for abdominal hysterectomy. EDG consists of Holter electrocardiography and photoelectrical plethysmography which are able to offer the values of Pre-ejection Period (PEP), Left Ventricular Ejection Time (LVET), and PEP/LVET (P/E), as well as prescribed conventional Systolic Time lntervals. PEP, LVET and P/E were measured together with blood pressure, heart rate, blood loss, urinary output, plasma electrolytes (Na and K), hematocrit (Ht), and plasma protein (TP) at pre-infusion period, during infusion, and 30 min, 60 min, 90 min after infusion. The results were compared with each group of the expanders, respectively, between those of pre-infusion, during infusion and 30 min, 60 min and 90 min after infusion. In addition, each of three sorts of the plasma expanders investigated was compared to physiological salt solution (S). The LVET prolonged during infusion in D40LR, S-HES and HES compared with S, which may suggest increase of the stroke volume. The shortening of PEP and decrease in P/E were obsereved at 60 min and 90 min after infusion compared with the pre-infusion values, notably in S-HES. P/E decreased during infusion in HES and D40LR. It may indicate the increase in preload and improvement of cardiac pump function. Ht decreased significantly in S-HES, which may be caused by hemodilution due to infusion of Salinhes(R), in addition to blood loss. Each plasma expander, especially Salinhes(R), restored circulating blood volume and improved cardiac pump function in the presence of blood loss during surgical intervention. EDG was considered useful for noninvasive investigation of cardiac function during infusion of the plasma expander.
