Abstract | 「要旨」循環停止によりおこる代謝性アシドーシスに対するDCA(dichloroacetate)の治療効果についてイヌを対象として検討した. 麻酔導入後, 開胸し, 右心房バルーンの膨張と大動脈遮断にて5分間循環を完全に停止させた. (1)DCA群(n=6)では5%DCA 100mg・kg-1を, (2)対照群(n=6)では生理食塩水2ml・kg-1を循環再開直後に静脈内に投与した. 循環再開直後は両群で一過性に左室拡張期終末圧が上昇し, 30分後には循環停止前面に戻った. 動脈血pHは循環停止によりDCA群で, 7.40より7.30へと, 対照群では7.38より7.27へと低下していたが, その回復はDCA群で速やかになる傾向を示した. 乳酸とピルビン酸は循環停止により上昇したが, 循環再開後はDCA群で対照群に比較し, いずれも有意に低下した. 以上より循環停止に際して, 循環再開後にDCAを投与することは乳酸の代謝を促進し, 代謝性アシドーシスの改善に有効であると思われた. |

Abstract | We studied the effects of dichloroacetate (DCA) on metabolic acidosis after circulatory arrest. Twelve mongrel dogs were anesthetized and ventilated using nitrous oxide and oxygen. Five minute circulatory arrest was induced by inflation of right atrial balloon and cross clamping of the ascending aorta. After restoration of circulation, animals were assigned to receive 100 mg/kg of 5%DCA (DCA group, n=6) or 2 ml/kg of physiologic saline (control group N=6). There was no significant difference in hemodynamic parameters including heart rate, cardiac output and arterial pressure between the two groups. Left ventricular end-diastolic pressure rose temporally after restoration of circulation in both groups, indicating slight reduction of cardiac performance. Arterial pH and base excess showed a tendency to return to baseline values sooner in DCA group than in control group. Serum pyruvate and lactate levels were significantly lower in DCA group than in control group at 15, 30 and 60 min after the restoration of circulation. We concluded that DCA is effective to treat metabolic acidosis induced by circulatory arrest. |
