Abstract「要旨」カルシウム拮抗薬であるニカルジピンの肝循環及び肝代謝への影響をイヌを用い検討した. 麻酔はエンフルレン, 酸素, パンクロニウムで導入挿管, 1MACエンフルレン, 100%酸素で維持した. 麻酔安定後開腹し, 門脈, 肝動脈に超音波トランジットタイム血流計を装置, また, 肺動脈に心拍出量測定の目的でスワン・ガンツタテーテルを, 門脈採血のため門脈にソーンズカテーテルを挿入した. 各準備終了, 麻酔安定後, ニカルジピン1, 2.5, 5, 15, 30, 60μg/kg/minを各10分間点滴し5分間隔で各諸量を測定した. 肝酸素供給量及び酸素消費量は計算により求めた. 5-15μg/kg/min投与では心拍出量, 総肝血流量及び肝酸素供給量は増加傾向を示したが60μg/kg/minでは血圧, 心拍出量は低下し, 総肝血流量及び肝酸素供給量も減少した. 以上より, ニカルジピンの高濃度投与では肝血流量の低下に注意が必要と思われる.

AbstractEffects of a calcium entry blocker, nicardipine hydrochloride in a dose of 1, 2.5, 5, 15, 30 and 60 μg/kg/min on hepatic blood flow and hepatic metabolism were studied in eleven mongrel dogs during 1 MAC enflurane anesthesia. Portal venous blood flow and hepatic arterial blood flow were measured by ultrasonic blood flowmetry. Hepatic oxygen supply and consumption were calculated from blood flows and oxygen contents. Nicardipine decreased mean arterial pressure but increased cardiac index, especially in 1, 2.5, and 5 μg/kg/min doses significantly. Total hepatic blood flow and hepatic oxygen supply increased in high dose administration (5-15 μg/kg/min), however decreased significantly in the highest dose (60 μg/kg/min). There were not significant changes in hepatic oxygen consumption with each dose. It seemed that high dose of nicardipine should be used carefully in hepatic circulation and metabolism.
