| Abstract | 「要旨」全身麻酔への硬膜外麻酔の併用は術野からの疼痛刺激を遮断し, 交感神経系を抑制し, 術後の鎮痛にも役立つことから広く普及しつつある. さらに, これら利点を強調し, 体外循環を用いる心臓手術の際にも硬膜外麻酔の併用は有用であるとする意見もある. しかし, 心臓手術での硬膜外麻酔併用の有用性を検討するには以下の点にも特に留意せねばならない. すなわち, 1)体外循環のために大量に投与されるヘパリンによる硬膜外血腫発生の危険, 2)交感神経遮断にともなう循環抑制およびそれに対処するための大量輸液(輸血)あるいは強心薬・昇圧薬投与の必要性, 3)循環系への影響の調節性の悪さ, である. これらと考え合わせると心臓手術では, 一般手術の場合と異なり硬膜外麻酔の利点が欠点を上回らないと判断される. その併用は有用とはいえない. 「はじめに」現在, 大阪大学医学部附属病院の麻酔科においては全身麻酔と硬膜外麻酔の併用を呼吸器外科手術や腹部手術の際には積極的に行なっているが, 体外循環を用いる心臓手術には応用していない. これは, 硬膜外麻酔の種々の利点を否定するからではなく, リスクに対してそのベネフィットが充分まさっていると言い切れないからである. |

| Abstract | Epidural anesthesia can block painful stimulation from surgical field, ameliorate sympatheic activity and provide sufficient control of pain postoperatively. Because of these advantages, combined general-epidura anesthesiia is gaining greater acceptance during gerneral thoracic and abdominal surgery in Japan. In addition, some anesthesiologists advocate its use in cardiac anesthesia. However, the disadvantages of the epidural anesthesia in cardiac surgery under cardiopulmonary bypass are：1) Risk of epidural hematoma formation due to the large amount of heparin given prior to the bypass. 2) Circulatory depression induced by the sympathetic blockade, which often necessitates administration of vasoactive drugs and/or large amount of fluids. 3) Poor controllability, especially late onset of and slow recovery from its circulatory effect. Even though it is generally beneficial to the patients especially during thoracic or upper-abdominal surgery, the risks and the disadvantages of the epidural supplement seem to outweigh its benefits in the cardiac anesthesia. |
