Abstract | 「要旨」麻酔導入量のミダゾラム(0.2mg/kg)の心機能, 心筋酸素需給バランスに及ぼす影響を心収縮期時相(systolic time intervals:STIs)から検討した. 対象はASA 1の手術予定患者11名である. 前駆出時間係数(pre-ejection period inex:PEPI), 左室駆出時間係数(left ventricular ejection time index:LVETI), PEP/LVET(Weisser係数)および拡張期血圧時間係数(diastolic pressure time index:DPTI), 張力時間係数(tension time index:TTI)を計測した. ミダゾラムによる導入後, 動脈血圧の低下と心拍数の増加をみたが, PEPI, LVETIには変化はなかった. PEP/LVETIはミダゾラム投与の7分後に一時的な上昇, DPTI/TTIは投与の1〜4分に下降をみた. しかしながら, いずれの変化も正常範囲内であり, PEP/LVETを心機能の指標に, DPTI/TTIを心筋酸素需給バランスの指標と考えると, 導入量のミダゾラム心機能および心筋酸素需給バランスに悪影響を及ぼさないとの結論が得られた. |

Abstract | Effects of induction dose of midazolam, 0.2 mg/kg iv were evaluated by measurment of systolic time intervals in 11 ASA I surgical patients whose avaraged age was 35 years after the institutional approval and informed consent. Although a decrease of blood pressure and increase of heart rate was observed, preejection period index (PEPI) and left ventricular ejection time index (LVETI) were not changed by midazolam administration. The PEP/LVET ratio was increased 7 min after midazolam and the DPTI/TTI (diastalic pressure time index/tension time index) ratio was decreased during 1 to 4 min after the administration of midazolam. However, these changes were all within normal ranges. These results indicate that midazolam 0.2 mg/kg iv in healthy patients does not impare either cardiac performance evaluating from PEP/LVET or myocardial oxygen balance from DPTI/TTI. The present study also confirmed that measurement of systolic time intervals is useful to assess cardiac function in a noninvasive fashion in the field of clinical anesthesia. |
