| Abstract | 「要旨」Swan-Ganzカテーテル(PAC)を挿入した98例の臨床例で, 右内頸静脈, 左内頸静脈, 右尺側皮静脈を刺入部位とした際の右室圧及び肺動脈楔入圧が得られるまでの挿入距離を計測した. さらに, 胸部X線で12時間後のカテーテル先端め位置移動を観察した. 右内頸静脈, 左内頸静脈, 右尺側皮静脈から右室圧測定部位までは, それぞれ25.4±1.52, 27.8±1.19, 53.1±3.09cm, 肺動脈図入圧測定部位までは43.6±2.07, 47.6±1.74, 73.5±2.57cmであった. 右尺側皮静脈からの距離Yは身長Xと正の相関関係を示し, 右室圧測定部位までの挿入長はY=0.336X-0.719cm(r=0.77)であった. 肺動脈楔入圧測定部位までの挿入長Yは身長Xと相関し, Y=0.318X+22.62cm(r=0.88)であった. 12時間後の胸部X線上, 内頸静脈群ではその約80%が1cm前後の肺動脈末梢への移動であったが, 尺側皮静脈群では約75%で3cm以上の先進が認められパーマネントウエッジとなる可能性が高いと考えられた. |

| Abstract | The distances to the right ventricle and to the pulmonary artery from the site of insertion of a Swan-Ganz catheter, were messured in 98 patients during perioperative periods. The insenion sites were the right and left jugular veins and the right basilic vein. The distances from the insenion sites to the tip of the catheter in place in the right ventricle were：a) right jugular vein, 25.4±1.5 cm ; b) left jugular vein, 27.8±1.2 cm ; and c) right basilic vein, 53.1±3.1 cm. The distances to the pulmonary artey were 43.6±2.1 cm, 47.6±1.7 cm, and 73.5±2.6 cm, respectively. The distance Y from the right basilic vein to both the right ventricle and pulmonary artery showed a positive correlation with the patient's height (X) (Y=0.336X-0.719 cm (r=0.77)). Chest X-rays taken 12 hours after catheter insertion showd a 1 cm forward displacement of the catheter tip in the jugular vein, and more than 3 cm displacement in the basilic vein. These findings will be helpful in preventing too deep insertion of the catheters and possible untorward consequences related to inserted catheters. |
