Abstract「要旨」脳血管外科手術後患者で血圧管理に長期間ニカルジピン持続静注を施行した症例8例を対象として, ニカルジピン長期持続静注が交感神経系に及ぼす影響について, 循環動態, 血中カテコラミン濃度を指標として検討した. 血圧は投与開始6時間後以降低下し, 血圧の変動幅は3日後以降減少し, 心拍数は6時間後には増加し1日後以降回復した. 血中カテコラミン濃度は1日後以降漸減した. 3日後, 5日後のニカルジピン投与量あるいは血中ニカルジピン濃度と血中ノルエピネフリン濃度は正の相関傾向を示した. 以上より, ニカルジピンは, 投与量依存性に交感神経系を賦活する作用を有するが, 長期持続静注による血圧管理下では, 循環動態の改善により, 2次的に交感神経系の過緊張は緩和すると考えられた.

AbstractEffects of long-term nicardipine infusion on sympathetic tone were studied in 8 patients, who had been infused nicardipine for more than 5 days in order to control blood pressure. Blood pressure and heart rate were measured at 6 hour, on 1st, 3rd and 5th day after the beginning of nicardipine infusion. Plasma catecholamine levels were measured on 1st, 3rd and 5th day after that. Systolic and diastolic blood pressure decreased significantly from 6 hours to 5 days of nicardipine infusion. Range of systolic blood pressure in a day reduced markedly on 3rd and 5th day. At 6 hour heart rate increased significantly, but on 1st, 3rd and 5th day heart rate decreased to the level of preinfusion. Plasma concentration of epinephrine and norepinrphrine decreased significantly on 3rd and 5th day compared with on lst day. On 3rd and 5th day nicardipine infusion rate or serum concentration of nicardipine correlated well with plasma norepinephrine level. It was concluded that nicardipine might activate sympathetic system, but during long-term infusion nicardipine might decrease sympathetic tone by improvement of hemodynamics.
