Abstract「要旨」1989年に著者らはくも膜下出血患者と同様の心電図変化を起こす家兎の実験モデルを発表した. すなわち家兎の脳底槽内に0.1%PGF2αを0.5ml注入すると心電図変化を誘発し, 1.0%リドカイン0.5mlを同所に注入すると不整脈が消失する. この機序を解明するために本実験では家兎18羽について交叉熱電対電極を脳底部に刺入しPGF2α(13羽)およびリドカイン注入時(5羽)の局所脳血流を連続記録した. PGF2α注入によりまず徐脈, ついで不整脈等の心電図変化を生じ, 血圧が上昇し局所脳血流が増加した. リドカイン注入で不整脈は直ちに消失したが局所脳血流は一定の傾向を示さなかった, くも膜下出血の心電図変化の機序として視床下部付近の貧血があげられているが, 本実験の結果から他の機序が考えられ, またリドカインの効果は脳組織への直接作用が考えられた.

AbstractIn 1989 we developed a new model of electrocardiographic (ECG) changes seen in patient with acute subarachnoid hemorrhage in rabbits. In this model, injection of 0.5 ml of 0.1% PGF2α into the basal cistern produced ECG changes, which were normalized with intracisternal administration of 0.5 ml of 1.0% lidocaine. In this study, to elucidate the cause of these ECG changes, regional cerebral blood flow (rCBF) near the basal cistern was measured con. tinuously using crossed thermocouple type texture blood flow measuring system in 18 rabbits. Intracisternal injection of PGF2a produced bradycardia followed by ECG changes. blood pressure rose, and the rCBF increased. Intracisternal injection of lidocaine immediately abolished the ECG changes, however it did not show any tendency for the rCBF. Recently, some authors stress that ischemia of the hypothalamus may produce arrhythmias. From this study we concluded that the ischemic changes of the area near the basal cistern is not the only cause of the ECG changes. The ECG changes were considered to be normalized by lidocaine with effect on the brain cells.
