Abstract | 「要旨」 虚血心筋における糖代謝の指標を求める為に虚血性心疾患患者30名を対象としF-18FDGを用いたポジトロンCT(PET)を施行し糖代謝量をPatlak法により定量的に求めた. この結果から血糖値, 酸素分圧比をパラメータとする糖代謝の指標を提案した. またインスリン, 速度定数からglucose transporterのactivityの変化に対する考察を行い虚血心筋における糖代謝書下のメカニズムを検討した. その結果虚血心筋の糖代謝の亢進は血糖値および心筋内の酸素消費量と酸素供給量のバランスで決定されていることが示唆された. また低インスリン時にはインスリン以外の物質でglucose transporterのactivityを高めて行われている事が示された. 速度定数からの検討では糖代謝を亢進するためにglucose transporterのactivityを高めるだけでなくリン酸化速度も高めて行われていることが示された. |

Abstract | In order to analyze change in the glucose metabolism in ischemic heart muscle, we evaluate regional glucose utilization rate in 30 patients with ischemic heart diseases by using F-18 FDG and PET. We have quantitatively measured the amount of the glucose metabolism with Patlak method. From the result obtained we would propose first an index for amount of the glucose metabolism with parameter consisting of blood sugar and oxygen partial pressure ratio. Secondary, we observed insulin level and rate constant by curve fitting method. Thus, we have discussed the mechanism of the glucose uptake in ischemic heart muscle. As a result, it was shown that the glucose uptake in the ischemic heart is depend on the balance of blood sugar and oxygen amount. On the other hand, it was also shown that in case with low insulin level susceptibility of the membrane would be likely strengthened by the substance other than insulin. Considering the rate constant obtained it was indicated that in order to keep glucose uptake, not only the susceptibility of the membrane but also the rate of phosphorylations must have been enhanced. |
