Abstract「要旨」非侵襲的に心拍出量を測定できる, 小児用Trans Tracheal Doppler(TTD)心拍出量計を用い, 仙骨硬膜外麻酔を受けた13人, 硬膜外麻酔を受けた5人, 吸入麻酔を受けた7人の1才から6才までの小児について, 術中の血圧, 心拍数, 心拍出量の変化を調べた. 吸入麻酔群, 硬膜外麻酔群では麻酔中循環系に明らかな変化は認められなかった. 仙骨硬膜外麻酔群の平均動脈圧には変化が認められなかったが, 心拍数は1%メピバカイン1ml/kg注入30〜60分後に有意の低下を示し, 40〜45分後には心拍出量が有意の低下を示した.

AbstractThe Hemodynamic effects of caudal anesthesia, epidural anesthesia and inhalation anesthesia were studied in 25 children aged beween 1 and 6 years. 1% mepivacaine was used for caudal anesthesia and epidural anesthesia. Cardiac output and cardiac index were measured with the transtracheal deppler method. Heart rate, mean arterial pressure and cardiac index remained unchanged in the epidural anesthesia group and the inhalation anesthesia group. However, after caudal anesthesia, we observed a significant decrease (p<0.01) in heart rate and cardiac index. It is apparent from these results, that precise evaluation and management of hemodynamics during caudal anesthesia are essential.
