Abstract「要旨」実験的に冠動脈狭窄犬を作製し, 胸部下行大動脈遮断時の血圧上昇に対し, 循環動態及び心筋組織血流量, 心筋代謝に及ぼす影響を, ニトログリセリンと比較検討した. ジルチアゼムにより, 心拍数は有意に低下し, 心拍出量は有意に増加した. しかし, 中心静脈圧, 肺動脈圧, 肺動脈楔入圧は有意に増加していた. ニトログリセリンでは, 心拍出量の有意な増加が見られたが, 心拍数は変化がなかった. また, 中心静脈圧, 肺動脈圧は対照値に回復したが, 肺動脈楔入圧は有意に増加していた. 内膜側の心筋組織血流量は, 両薬剤投与により有意に増加した. 心筋乳酸摂取率は, ジルチアゼムでは有意に低下した. 一方, 心筋酸素摂取率は, ジルチアゼムでは変化がなかったが, ニトログリセリンでは有意に上昇した. ジルチアゼムは速やかな降圧作用を示し, 心筋酸素需給バランスを保ち, 大動脈遮断時の降圧剤として有用であることが示唆された.

AbstractThe effects of diltiazem and nitoroglycerin on cardiovascular hemodynamics and myocardial blood flow (MBF) were investigated in anesthetized dogs with normal and experimentally produced ischemic heart during aortic cross clamp. The myocardial ischemia was produced the ischemic area by ligation on left anterior descending coronary artery and was devided into the intact (MBF>100 ml/min/100 g) and ischemic (MBF<100 ml/min/100 g) areas. MBF was measured in each area by hydrogen clearance method. 1) Diltiazem produced significant decrease in heart rate and both drugs produced significant increase in cardiac output. 2) Both drugs produced significant increase in MBF in inner layer of myocardial wall in normal and ischemic heart. Diltiazem produced significant increase in inner/outer ratio at 5 min after aortic cross clamp in normal heart. 3) Nitroglycerin produced significant increase in myocardial oxygen extraction and diltiazem produced significant decrease in myocardial lactate extraction, These results suggest that diltiazem is useful for hypertension during aortic cross clamp.
