Title | 実験的心筋梗塞犬における, セボフルレン-笑気麻酔の循環動態ならびに心筋代謝に及ぼす影響 |

Abstract | 「要旨」雑種成犬15頭を用い, 1.5%及び3.0%セボフルレン50%笑気麻酔を行ない, 正常心ならびに冠動脈前下行枝を結紮した虚血心について, 循環動態と心筋代謝に及ぼす影響を測定した. 正常心においては, セボフルレン1.5%, 3.0%いずれの吸入濃度においても濃度依存性に心拍数, 平均動脈圧, 心拍出量, 心筋組織血流量の低下がみられたが体血管拡張作用はみられなかった, 心筋酸素摂取率, 心筋乳酸摂取率, 冠静脈洞乳酸・ピルビン酸比は変化しなかったが, EVRは1.0以上でむしろ濃度依存性に増加し, 心筋代謝は保たれていると考えられた. 虚血心においては, セボフルレンの吸入濃度依存性に平均動脈圧の低下が見られたが, 心拍数, 心拍出量共に有意な低下は示さず, このため3.0%セボフルレンでは, 心筋抑制作用と同時に体血管拡張作用が見られた. 心筋組織血流量は全領域に渡り, 3.0%セボフルレンで有意の低下を示した. EVRは3.0%セボフルレンでわずかに減少したが, 有意な低下ではなかった. 心筋酸素摂取率, 心筋乳酸摂取率, 冠静脈洞乳酸・ピルビン酸比は変化しなかった. 以上よりセボフルレンは虚血心において, 1.5%では心筋に及ぼす影響は少なく, 3.0%では軽度の心筋抑制を示したが, 心筋酸素需給バランスは保たれていると考えられた. |

Abstract | We investigated the effects of sevofiurane-nitrous oxide anesthesia on hemodynamics and myocardial metabolism in experimentally produced myocardial ischemia by ligation on the left anterior descending coronary artery in dogs. In normal heart, sevofiurane produced a dose-related decrease in heart rate (HR), mean arterial pressure (MAP) and cardiac output (CO) but systemic vascular resistance (SVR) was unchanged. Left ventricular stroke work (LVSW) decreased and EVR (DRTI/TTI) was well maintained. Myocardial metabolism was unchanged. It was suggested that myocardial oxygen demand-supply was maintained. In ischemic heart, like normal heart, sevoflurane produced a dose-related decrease in MAP and LVSW, but HR and CO did not decrease significantly. Corsequently in concentration of 3%-sevoflurane. SVR was decreased. EVR, in concentration of 3%-sevoflurane, did not decreased significantly and myocardial metabolism was unchanged. In any case, myocardial oxygen demand-supply was maintained. It is concluded that, in ischemic heart, changes of hemodynamics and myocardial metabolism during 1.5%-sevoflurane is minimum but there is a possibility to cause myocardial ischemia during 3%-sevoflurane. |
