Abstract | 「要旨」開心術時の胸骨切開, 体外循環による血中カテコラミン濃度増加に及ぼすミダゾラムの影響について, 弁置換術, 冠動脈バイパス術症例を対象とし, コントロール群, ミダゾラム群各7例で検討した. フェンタニール30μg/kg, ミダゾラム0.1mg/kgで麻酔導入し, 胸骨切開前に, フェンタニール10μg/kgに加え, ミダゾラム0.1mg/kgを投与することにより, 胸骨切開による血中カテコラミン濃度増加が抑制された. しかし, 本投与法では, 体外循環による血中カテコラミン濃度増加は抑制できなかった. |

Abstract | In open heart sergery serum catecholamine levels were elevated at sternotomy and during extra corporeal circulation (ECC). The effects of midazolam on catecholamine responses during cardiac surgeries were studied. Fourteen patients were divided into 2 groups, control group and midazolam group. In both groups anesthesia was induced with midazolam 0.1mg/kg, fentanyl 30μg/kg and vecuronium 0.1mg/kg. Before sternotomy, fentanyl 10μg/kg was given in both groups and midazolam 0.1mg/kg was administered only to midazolam group. There were no significant differences between 2 groups in blood pressure and heart rate. But during sternotomy, blood pressure in midazolam group increased less than control group. After sternotomy serum concentrations of epinephrine and norepinephrine increased significantly in control group, but not in midazolam group. During ECC serum catecholamine levels in both groups increased significantly and no difference was seen between 2 groups. It was concluded that midazolam decreased catecholamine response to sternotomy but not to ECC. |
