Abstract「要旨」抱合型noradrenaline(NA)が遊離型NAに転換される場と転換の促進因子に検討を加えた. 虚血性心疾患患者にて, (1)安静時, 遊離型NA濃度は, 冠静脈洞血>大動脈血, 抱合型NA濃度は冠静脈洞血<大動脈血を示し, 総NA濃度は両血液間に差を認めなかった. (2)下肢動的運動負荷時, 肘静脈血と肺動脈血において, 遊離型NA濃度は増加, 抱合型NA濃度は減少し, その増加量と減少量は, 肺動脈血>肘静脈血を示した. このとき, 肺動脈血のpHは有意の低下を示した. (3)全血(100ml)に脱抱合酵素であるarylsulfataseを添加した際, pH7.4の条件下にて, arylsulfataseの1mgは遊離型NA濃度を増加, 抱合型NA濃度を減少せしめたが, 0.1mgは両NA濃度に有意の変化を与えなかった. しかし, pH7.1にて, arylsulfataseの0.1mgは遊離型NA濃度を増加, 抱合型NA濃度を減少せしめた. 以上の成績から, 虚血性心疾患患者に運動負荷を加えたとき, 心臓において, 抱合型NAの少なくとも一部は遊離型NAに転換され, この促進因子の1つとして, 血液pHの低下が関与する可能性が示唆された.

AbstractWe investigated the site and the precipitating factor of deconjugation of plasma conjugated noradrenaline (NA). We compared the plasma levels of free and conjugated NA simultaneously at the coronary sinus and the aorta in patients with ischemic heart disease (n=10). We also measured plasma levels of free and conjugated NA at the antecubital vein and the pulmonary artery simultaneously before and during dynamic leg exercise in cardiac patients (n=17). Furthermore, we investigated the effects of arylsulfatase (0.1 mg, 1 mg) on plasma levels of free and conjugated NA in the 100 ml of whole blood from healthy subjects (n=16) under condition of physiological (7.4) or low (7.1) pH in blood. 1) Plasma level of free NA was higher in the coronary sinus than in the aorta. Plasma level of conjugated NA was lower in the coronary sinus than in the aorta. There was no significant difference in plasma level of total NA between the coronary sinus and the aorta. 2) Plasma level of free NA increased and plasma level of conjugated NA decreased during exercise in both the antecubital vein and the pulmonary artery. The increase in plasma free NA and the decrease in plasma conjugated NA were both greater in the pulmonary artery than in the antecubital vein. 3) Under condition of pH 7.4 in blood, the addition of 1 mg of arylsulfatase led to a significant rise in plasma NA level and a significant decrease in plasma conjugated NA level but 0.1 mg of arylsulfatase hardly affected plasma level of free or conjugated NA. However, 0.1 mg of arylsulfatase led to a significant rise in plasma NA level and a significant decrease in plasma conjugated NA level under condition of pH 7.1 in blood. These findings suggest that, in patients with ischemic heart disease, plasma conjugated NA can be deconjugated, at least in part, in the heart and that one of the precipitating factors of deconjugation is the low pH in blood.
