| Abstract | 「要旨」慢性心不全症において運動耐容能と酸素輸送能の関係を検討することを目的に, NYHA I°9例, II°17例, III°12例の計38例に観血的心肺運動負荷試験をindividual ramp protocolで行った. [結果] peak VO2と peak DO2は r＝0.91(P＜0.001)の有意な相関を示した. また peak DO2は peak Qと r＝0.94(p＜0.001), peak Qは peak SVIと r＝0.88(P＜0.001)の有意な1次相関を示した. [結論]重症度が広く分布した慢性心不全症においては運動時の酸素輸送能は好気的運動能を規定する因子であった. また酸素輸送能は心ポンプ機能に依存しており, 運動耐容能と心ポンプ機能の間には基本的な関連が存在することが示された. 「はじめに」慢性心不全症は心機能障害に運動能の低下, 心室性不整脈の多発, 予後不良の要素が加わった症候群1)とする考えがあるように, 運動耐容能の低下は慢性心不全症の臨床像を決定する重要な要因である. |

| Abstract | To elucidate the relationship between exercise intolerance and oxygen delivery in patients with chronic heart failure, 38 cases including NYHA I° in 9 cases, NYHA II° 17 cases and NYHA III° 12 cases underwent an invasive cardiopulmonary exercise tolerance test using individual ramp protocol. The peak VO2 correlated significantly with the peak DO2 (r=0.91, p<0.001). The peak DO2 correlated with the peak Q (r=0.94, p<0.001). And the peak Q correlated with the peak SVI (r=0.88, p <0.001), but did not correlate with the peak HR. Our results suggested that if the studied subjects include various severity of heart failure the limitted oxygen delivery makes a limited oxygen intake. And the oxygen delivery depends upon the cardiac pump function. Thus the impaired cardiac pump function was a cause for exercise intolerance in patients with chronic heart failure. |
