Abstract「要旨」体重18kg以上のファロー四徴症根治術を無輸血で試みた. 対象は, 麻酔中に無輸血で管理し得た無輸血群4例と輸血を要した輸血群5例とした. 無輸血群は全例体重が28kg以上であった. 出血量は無輸血群では400ml以下であったが, 輸血群では1000mlを越えるものが2例あった. CPB中のHbは5g/dl以下が無輸血群で1例, 輸血群で2例あった. CPB中Hbの安全限界に今後の検討の余地があった. 開心術を無輸血で行なう試みは, 多くの施設で実施されている. 著者らの施設でも体重30kg以上の心房中隔欠損症や心室中隔欠損症の根治術, A-Cバイパス術, 弁置換術を無輸血で行ない好成績を得ている1)2). また, 体重20kg未満の小児における無輸血開心術も報告されている3)4)5). 今回, ファロー四徴症の根治術を無輸血手術で試みた症例を経験したので, 検討し報告する.

AbstractWe attempted to conduct radical operations avoiding homoglogous blood transfusion for nine patients with tetralogy of Fallot, whose body weight were more than 18 kg. Four patients were operated without homologous blood transfusion (group NT). Five patients received homologous blood transfusion (group T). No critical signs were noted throughout the operation, whereas an averaged hemoglobin level fell down to 6.1 g/dl in the group NT during cardiopulmonary bypass (CPB), e.g. to 4.3 g/dl in one of the patients. Minimal hemoglobin level of 5 g/dl has been provided for a obtained in this study, however, the safety-limit of hemoglobin level should be better reconsidered in the near future.
