Abstract | 「要旨」心嚢内圧を自動的に一定圧にコントロールできるフィードバックコントロール装置を用いて実験的心タンポナーデモデルを作製した. 心嚢内圧を0mmHg, 3mmHg, 6mmHg, 9mmHgと上昇させ, 一定時間一定圧に維持し, 心拍数, 動脈圧, 心内圧, 心拍出量に加え, 左室右室の局所心筋長を連続的に記録し, 血行動態, 局所心筋機能の変化を観察した. 心タンポナーデの初期(心嚢内圧が3〜6mmHgのとき)には, 右室と左室の局所心筋機能に解離がみられた. 加えて, 従来の報告にみられるよりも低い心嚢内圧で血行動態の虚脱がみられ, モデルの違いによる影響が示唆された. |

Abstract | Cardiac tamponade model was created in anesthetized, acutely instrumented dogs using a closed-loop feedback control of intrapericardial pressure. Hemodynamic variables and right and left regional myocardial functions were measured with the intrapercardial pressure maintained at either 0mmHg, 3mmHg, 6mmHg or 9mmHg for a fixed period. Hemodynamic derangement occurred at lower intrapericardial pressure of 9mmHg than those reported previously. Futhermore, right and left regional myocardial functions dissociated each other at lower levels of intrapericardial pressures of 3-6mmHg than those that caused cardiovascular collapse, We suggest that these observed characteristics were mainly due to the differences of animal models. Further study may be warranted to link the observed phenomenon to clinical significance. |
