| Abstract | 「要旨」内頚静脈への中心静脈カテーテル挿入を現在より簡単, 確実, 安全そして迅速にするためにSeldinger法を用いた独自の穿刺キットを試作した. これは22ゲージ(G)の試験穿刺針のハブに側孔を作り, そこから細いガイドワイヤーを挿入できるようにしたもので, 試験穿刺のみでガイドワイヤーの挿入が可能である. これを成人10例に使用したところ, ガイドワイヤーにも充分な強度があり, 全例で容易に中心静脈カテーテルを挿入でき, 極めて良好な結果が得られた. 穿刺に伴う合併症は認められなかった. 「はじめに」近年, 術中術後管理に中心静脈カテーテルや肺動脈カテーテル(Swan-Ganzカテーテル)を使用する症例が増加している. その穿刺部位として内頚静脈が多く用いられており, 穿刺法にも数々の工夫が報告されている1)〜7). しかし時折, 試験穿刺には成功しても, その後の太い穿刺針では失敗し, カテーテル留置が困難となった症例を経験した. |

| Abstract | We have developed a new kit for internal jugular vein cannulation to make the procedure safer, easier and simpler. A small gauge (22 gauge) metal pilot needle is equipped with a Y-shaped hab whose side-port accepts a small gauge (0.46mm) Seldinger guidewire. Once the needle hits the vein, it is held steady in place and the guidewire is threaded in through the side-port. there is no need to remove the pilot needle and repeat a skin puncture with a larger-bore needle for introduction of guidewire. this maneuver is conductive to difficulty and complications because of compression and dislocation of the vein. With our method a single puncture with a sharp, small needle achieves the location of the vein and introduction of the guidewire in one step. In a small sense of first ten cases, cannulation was achieved on the first attempt in all cases quickly and without complications. |
