| Abstract | 「要旨」甲状腺機能低下症を合併した患者5例の開心術術後管理を経験した. 術後早期は甲状腺剤を使用せず混合静脈血酸素飽和度(以下SvO2)の連続モニターと血中乳酸値の随時測定を行いながら管理した. 心係数は低いもので, 2.01/min/m2前後であったが, 良好なSvO2と乳酸値を示しており, 術前より他の合併症のないものは順調な経過で当院ICUを退室した. 甲状腺機能低下症と心機能の関係において, 開心術術後患者の様にある程度心機能が制限されているような場合, SvO2や乳酸値の測定を行いながら, 甲状腺機能低下状態での酸素の需要と供給のバランスを保てば, 心係数がかなり低い場合でも管理可能と考えられた. 「はじめに」甲状腺機能低下症を有する心疾患患者の管理では, 甲状腺ホルモンの不足は心拍出量の低下や, 循環不全の原因となり, 他方甲状腺剤の投与は, 組織での酸素消費量を増大させて狭心症発作を誘発したり1), 心への負担が増大し, 心機能悪化の要因ともなり, そのコントロールは難しいとされている2). |

| Abstract | We reported the postoperative treatment of five patients who received open heart surgery and consequently developed hypothyroidism. In the early postoperative days, rather than treating patients by intravenous administration of thyroidal preparation. The patients generally showed low cardiac index (CI) , but showed a favorable SvO2 and lactic acid level in blood. Those patients who had no complication were discharged from the intensive care unit after running a favorable course. The results of our study suggest that it is practicable to treat a postoperative patient with even a considerably low CI by monitoring the SvO2 and blood lactic acid level, in such cases if the balance between the supply and demand of oxygen is maintained adequately during hypothyroidism. |
