Abstract「要旨」成人開心術症例においてPGE1を体外循環中20ng/kg/min, 体外循環終了後術後1日目まで10ng/kg/minで投与したPGE1群(10例)とコントロール群(10例)を対象としてPGE1投与が体外循環による腎障害に及ぼす影響をN-acetyl-D-glucosaminidaseインデックス, ナトリウム排泄率, クレアチニンクリアランス, 血清ヘモグロビン濃度を指標として検討した. 体外循環により尿細管細胞, 尿細管機能および糸球体濾過は障害され, 溶血が起こった. いずれの指標においても両群間に有意な差を認めなかった. PGE120ng/kg/minの静脈内投与では, 体外循環中および体外循環後の腎障害に対してPGE1による明らかな腎保護作用は認められなかった.

AbstractWe studied the effects of Prostaglandin E1 (PGE1) infusion on renal injury during and after cardiopulmonary bypass (CPB) in 20 adult patients undergoing cardiac surgery. These patients were divided into two groups:control group (n=10) and PGE1 group (n=10). PGE1 was infused intravenously at the rate of 20ng/kg/min during CPB and 10ng/kg/min after CPB until postoperative day 1. N-acetyl-D-glucosaminidase (NAG) index, fractional sodium excretion (FENa), creatinine clearance (Ccr) and serum hemoglobin concentration were measured before induction of anesthesia, during CPB (30, min after initiation of CPB and 20 min after aortic declamping) and after CPB (15 min and 3-4 hr after termination of CPB and postoperative day 1, 3-4, and 7). In both groups, NAG index, FENa and serum hemoglobin concentration increased, while Ccr decreased, and the changes of these parameters showed no significant differences between two groups during and after CPB. Continuous administration of PGE1 during and after CPB could not demonstrate the apparent renal protective effects in the patients undergoing cardiac surgery.
