| Abstract | 「はじめに」Marfan症候群は, 1896年フランス人医師Marfanにより初めて報告された先天性結合織代謝異常症である. 今回われわれは, 解離性大動脈瘤を有する本症候群患者の腹式帝王切開術の麻酔を経験したので報告する. 「症例」29歳, 女性. 身長182cm. 体重−非妊娠時56kg, 妊娠38週時71.5kg, 高等学校時代は, バレー部に所属. 23歳時に他院にてMarfan症候群を指摘された. 23歳で結婚, 25歳時に第1子を満期正常分娩したが, この妊娠中は著変なく, 出産時陣痛促進剤を使用した以外は正常な妊娠分娩経過であった. 家族歴 父親が40歳, 父親の同胞2名がそれぞれ63歳, 30歳で, 父方の祖母も60歳代に心疾患で死亡している. 第1子もMarfan症候群の疑いで経過観察中である. 現病歴 妊娠35週3日目, 夜間就寝中に, 前胸部圧迫感, 背部痛, 呼吸困難, 両手痺れ感を自覚. 痛みは数秒で半減したがその後も持続するため, 翌朝本院を受診し, 精査のため入院となった. 入院時, arm spanが身長とほぼ等しく, くも状指(図1), 外反肘, 水晶体亜脱臼がみられた. 入院時血圧は150/80mmHgであった. 検査所見では, 血算, 肝機能検査, 腎機能検査, 血液ガス所見に異常値はみられなかった. |

| Abstract | A 29-year-old female with Marfan's syndrome complained of chest pains at 35-week's gestation. She had aortic arch dissection, and Cesarian section at 38-week's gestation was scheduled. Under spiral anesthesia. Reasons we chose it are；1) In the case of general anesthesia, vital signs vary greatly with induction and recovery of anesthesia, 2) In the case of spinal anesthesia, induction time is quicker than epidural anesthesia, 3) In our hospital we usually choose spinal anesthesia in Cesarian section. In this case, both blood pressure and heart rate were relatively stable during and after Cesarian section. |
