Abstract「要旨」Goldenhar症候群不全型の1才10ヶ月女児の形成外科手術の麻酔を経験した. 明らかな小顎症はなかったにもかかわらず盲目的気管内挿管を必要とした. 本症候群では, 顔面奇形, 脊椎奇形, 心血管奇形等を合併するため, 気道および循環器合併症に関する詳細な術前評価と臨床状況に応じた適切な気道管理を含む慎重な麻酔管理が必要である. 「はじめに」Goldenhar症候群は眼の奇形に耳, 下顎, 脊椎の奇形を伴った複合奇形症候群であり, 胎生期の第1第2鰓弓の形成異常によって生じる1〜3). 麻酔管理上, その解剖学的原因による気道確保の困難が問題となるが4), 日本における本症候群の麻酔に関する報告は少ない8〜10). 今回は本症候群の不全型患者における巨口症, 副耳の手術の麻酔を経験したので, 若干の文献的考察を加えて報告する. 「症例」1才10ヶ月の女児で身長は80cm, 体重は10kgであった. 骨盤位のため帝王切開術にて出生した. 生下時体重は2604gで出生直後より眼瞼の脂肪類皮腫, 副耳, 巨口症が認められ, 今回, 副耳, 巨口症に対する形成術が予定された.

AbstractOne year and 10 months old girl with Goldenhar's syndrome is presented who underwent the plastic surgery of facial abnormalities. Although the patient did not show typical micrognathia, blind endotracheal intubation was required. The pre-operative assessment of difficulty in intubation is essential. Induction, maintenance and emrgence of anesthesia for the patient went uneventful. Cardiovascular and craniovertebral anomalies should be also considered for anesthetic management of patients with Goldenhar's syndrome.
