Abstract「要旨」ネコを用いて, 脳静脈還流路を物理的に遮断した状態(A群)と頭蓋腔に静水圧が負荷されただけの頭部低位状態(B群)とで, それぞれ同程度の頭蓋内圧(ICP)亢進となるような二群の脳静脈鬱血のモデルを作製した. 両群間の頭蓋腔の圧−容量関係と脳脊髄液吸収動態の差異を, 頭蓋腔コンプライアンス(C), pressure-volume index(PVI), 脳脊髄液流出抵抗(Ro)などの指標を用いて比較し, あわせて占拠性病変による基本的なICP亢進モデル(C群)との相違を検討した. さらに, cranial window法による脳表血管の形態観察から, 二つの静脈還流障害における脳血管床の反応態度の差異も検討した. その結果, 各群とも実験操作によるICP負荷が約10mmHgと同程度であるにもかかわらず, C, PVIは実験操作前の対照値に比較して, A群では不変, B群では増加, C群では減少した. また, RoはA群, C群では増加したが, B群では減少し, A群およびB群の両静脈還流障害モデル間での頭蓋内圧環境に大きな相違があることが明らかとなった. さらに, 脳軟膜血管の形態反応にも差がみられ, 頭蓋内圧環境が相違する一因として脳血管床の果たす役割が示唆された.

AbstractWe established two different cat models of cerebral venous congestion increasing intracranial pressure (ICP) to similar degrees by physical interruption of the cerebral venous return (Group A) or low head position in which hydrostatic pressure is applied to the cranial cavity (Group B). The pressure-volume relationship in the cranial cavity and changes in cerebrospinal fluid (CSF) absorption were compared between the two groups using parameters such as intracranial compliance (C), pressure-volume index (PVI), and CSF outflow resistance (Ro). In addition, differences between these models and basic model of intracranial hypertension due to space-occupying lesions (Group C) were evaluated. Differences in the response of the brain vascular bed were also investigated between the two models of venous return impairment by observaton of the morphology of blood vessels in the brain surface using the cranial window method. Though the ICP load was about 10 mmHg in all groups, C and PVI were unchanged in Group A, increased in Group B, and decreased in Group C. Ro increased in both Group A and C but decreased in Group B. These results suggested that there were marked differences in ICP dynamics between the two models of venous return impairment. In addition, the morphological responses of the pial vessels also differed, suggesting that the differences in ICP dynamics are associated with the cerebral vascular bed. (Circ Cont 16:331〜339, 1995)
