Abstract | 「要旨」セボフルラン麻酔下における脳血管の二酸化炭素(CO2)反応性について, 経頭蓋骨的超音波ドップラー法(TCD)を用いて検討した. 20〜70歳の開腹手術症例40例のうち, 術中, 呼気終末CO2濃度(EtCO2)を20〜50mmHgまで変化させ, EtCO2 5mmHg毎にTCDにて左中大脳動脈平均血流速度(mV), Pulsatility Index(PI)を測定し得た15例で解析した. 血圧, 心拍数, 呼気セボフルラン濃度は各測定点間に有意差を認めなかった. 20〜44歳の7例と45〜70歳の8例に分けると, EtCO2が40mmHg以上で若年群が高齢群より有意に大きいmVを示したが, PIには有意な群間差を認めなかった. 以上より, セボフルラン麻酔下での脳血管のCO2反応性は若年者より高齢者で低下することが示唆された. 「緒言」人における脳血管の二酸化炭素(CO2)反応性に関しては, 近年, 経頭蓋骨的超音波ドップラー法(TCD)を用いて検討されている1,2)が, 血流速度のみの検討が多い. |

Abstract | Cerebrovascular CO2 reactivity (CCO2R) was investigated by measuring middle cerebral arterial blood flow velocity (Vm) using transcranial Doppler (TCD) in 15 patients undergoing abdominal surgery from 20 to 70 years old. Endtidal CO2 concentration (EtCO2) was changed from 20 to 50 mmHg by changing the respiratory rate. Vm and pulsatility index (PI) were measured at each 5 mmHg in EtCO2 by TCD. There were no significant changes in blood pressure, heart rate, EtS, or PI as the EtCO2 was increased from 20 to 50 mmHg. An increase of Vm was observed accompanying the increase in EtCO2. Its increase was significantly larger in younger patients (7 cases from 20 to 44 years old) than that in older patients (8 cases from 45 to 70 years old). It was concluded that under sevoflurane anesthesia CCO2R was preserved when EtCO2 was changed from 20 to 50mmHg, but it was greater in the 20 to 44 age group than in the 45 to 70 age group.
