Abstract | 「要旨」成人開心術167例に対し人工心肺離脱時のアムリノン投与例(投与群:85例), 非投与例(非投与群:82例)で併用投与が必要とされたカテコラミンの種類及び症例数とその血行動態に対して検討を行った. 人工心肺離脱直後の血行動態測定にて併用カテコラミンを決定し, 離脱1時間後と閉胸後に血行動態測定を行い, 比較検討を行った. 結果として閉胸後に投与群は非投与群に比べ平均血圧とSVRが有意に低値を, 心係数が有意に高値を示した. 併用カテコラミンはドパミン投与症例数は投与群が非投与群に比べ有意に少なく, ノルエピネフリン投与症例数は投与群が非投与群に比べ有意に多かった. 従って人工心肺離脱時のアムリノンの投与はドパミンの使用頻度を減少させるが, ノルエピネフリンの使用頻度を増加させ, アムリノンの持つ血管拡張作用, 心拍出量増大作用が閉胸によっても影響されにくいと考えられた. |

Abstract | The study was done on 167 patients who received open heart surgery to evaluate the effect of amrinone on hemodynamics after cardiopulmonary bypass (CPB). Patients were divided into two groups;group I was infused amrinone concomitantly with catecholamine (85 patients);and group II was treated with catecholamine only (82 patients). Amrinone was administerd as a single dose of 1.5mg・kg-1 into the venous reservoir of the CPB circuit 15 min prior to thd end of CPB, followed by an continuous infusion of 5 μg・kg-1 min-1. According to the hemodynamics measured just after the end of CPB (after-0) combined effects of amrinone with catecholamines were determined. Thereafter, hemodynamics were measured at one hour after the termination of CPB (after-1) and after chest closure (after-2). In group I mean blood pressure and systemic vascular resistance at after-2 were significantly lower than those of group II, and cardiac index at after-2 was significantly higher than that of group II. The rate pressure product showed no significant difference between two groups. The frequency of dopamine use was reduced, and the frequency of norepinephrine use was increased by amrinone administration.
