Abstract「要旨」われわれは, 冠動脈バイパス術予定手術患者20例を対象に, 血漿中カルシトニン遺伝子関連物質(CGRP)の人工心肺中の変動について検討した. 同時にエピネフリン, ノルエピネフリン, 抗利尿ホルモン, 心房性ナトリウム利尿ホルモンの測定も行った. 血漿CGRPは人工心肺開始直後, 大動脈遮断前に麻酔前値(11.1±1.2pg/ml)の約10倍(109.1±6.3pg/ml)に上昇し, 遮断30分後に有意に低下した(41.6±6.3pg/ml). CGRPの動きは同時に測定したホルモンやアミン類とは異なる動きを示した. CGRPの動きは平均血圧, 梢血管抵抗の動きと相反したが, 血行動態とCGRPの間には相関を認めることができなかった. CGRPの人工心肺開始時の上昇を報告した初めての報告であるが, 人工心肺開始時の上昇機転に関しては, 本研究の成績から十分に説明し得なかった.

AbstractTo evaluate the effects of cardiopulmonary bypass (CPB) on the level of calcitonin gene-related peptide (CGRP), we measured plasma epinephrine, norepinephrine, antidiuretic hormene, human atrial natriuretic hormone, simultaneously with CGRP. Plasma CGRP increased significantly 10 times more with initiation of CPB before cross clamping of aorta compared with that of pre anesthesia, and decreased significantly 30 minutes after cross clamping of aorta. This increase was not associated with the change in epinephrine, norepinephrine, antidiuretic hormone and human atrial natriuretic hormone. Though the increase showed reciprocal pattern with the change of blood pressure and peripheral vascular resistance, we cold not find relationship statistically. We believe that the present result is the first observation that there was significant increase of CGRP during CPB, though the mechanism remains unknown. (Circ Cont 17:214〜222, 1996)
